Reminder: Returning this application does not guarantee a performance slot in the festival dance showcase.  If selected, you will receive your confirmation of a showcase performance slot by April 1^st.

This form must be completely filled out along with a check or money order payment (if applicable) in order for your application to be considered.  Applications for the festival stage must be received by 3/1/2010 for consideration (festival stage deadline has been extended, submit your registration to perform today!).

Refund Policy: Cancellations received in writing, at least 15 days prior to the event, will receive a refund less a $25 cancellation fee, provided we can find a substitute vendor or presenter. Written notification must be sent directly to the CADA via email: info@artsanddance.org, or Cultural Arts and Dance Association, PO Box 6344, Hayward, CA 94540-6344.
