Payment Procedure:  Make checks payable to Cultural Arts and Dance Association.  Please enclose a check or money order for the Total Payment Due.   Mail your payment, application and self addressed stamped envelope to:  Angel Thacker, CADA, PO Box 6344, Hayward, CA 94540-6344.

 

Application Agreement:  By registering for this event vendors agree to all terms listed below.  Vendors will supply all materials for set up, display and/or demonstration.  Space allotted is at the discretion of the Event Coordinator.  Some electricity will be available.  No generators please.  Food vendors are required to obtain the proper permit from the Santa Clara County Health Department.  Sales Tax collection and payment is solely the responsibility of each vendor.  No pets on property please.  Set up must be completed and vehicles removed from festival area by 9:30am on event day.  No break down of your display may begin before 6:45 pm.  All vendor spaces must be occupied at all times during the event.  Neither the Cultural Arts and Dance Association nor the Hoover Theater are responsible for any losses in sales or inventory due to weather or other incident.  The Festival reserves the right to deny participation without registration refund to a vendor who displays inappropriate items for this event

 

The undersigned, in consideration of participation in this event, agrees to indemnify and hold harmless, and release the Cultural Arts and Dance Association from any and all liability for any injury or loss which may occur by the above named business and/or individual registered to sell at the World Dance Celebration.
